Form I-94 Replacement (Lost or Stolen)

 

F-1 student has the obligation to keep his or her Form I-94 (also known as “the little white card in passport”) in a safe place. We recommend that it remain stapled inside your passport. However, if a Form I-94 is lost or stolen for any reason, a student must apply for Form I-94 replacement immediately.  This form is critical to verifying your status as an F-1 student as that is the form which confirms that you entered the US in that status.


Required Documents (all must be valid and original copy) at the USCIS:

  • Form I-102 (download at http://www.uscis.gov/graphics/formsfee/forms/i-102.htm)
  • Make check or money order payable to U.S. Citizenship and Immigration Services
  • Copy of current Form I-20
  • Copy of Form I-94 (if available)
  • Copy of passport (biographical information page)
  • Copy of Police Report (if stolen)

What You Need To Do:

  • Step 1: Mail all required documents to the USCIS California Service Center (see address below)
  • Step 2: You will receive a receipt notice from USCIS within a few weeks
  • Step 3: Once you receive your new Form I-94, bring it to ISPS for photocopy

Required Documents (all must be valid and original copy):

Note 1: Student may be allowed to depart from the U.S. without Form I-94. However, a student may face difficulty when he or she returns to the U.S. in the future (with any type of visa)

Note 2: Without Form I-94, a student may not apply for any immigration benefits including OPT, reinstatement, automatic visa revalidation, accept new employment, etc
