J-1 student has the obligation to keep all Forms DS2019 in a safe place. However, if a Form DS2019 is lost, stolen, or damaged for any reason, a student must apply for its replacement immediately. Required Documents (all must be valid and original copy) at ISPS: - Document Request Form
- Personal statement explaining circumstances in which document was lost, stolen, or damaged
- Form DS2019 (if damaged)
- Police Report (if stolen)
What You Need To Do:- Step 1: Submit all required documents to the ISPS front desk
- Step 2: Wait for email from ISPS (if you don’t hear from ISPS after 10 business days of submission, call at 621-4627 to check your application status)
- Step 3: Pick up your new DS2019 at the ISPS front desk
