Immigration : J1 : Form DS2019 Replacement (Lost, Stolen, or Damaged)

Form DS2019 Replacement (Lost, Stolen, or Damaged)

 

J-1 student has the obligation to keep all Forms DS2019 in a safe place. However, if a Form DS2019 is lost, stolen, or damaged for any reason, a student must apply for its replacement immediately. 

Required Documents (all must be valid and original copy) at ISPS:

  • Document Request Form
  • Personal statement explaining circumstances in which document was lost, stolen, or damaged
  • Form DS2019 (if damaged)
  • Police Report (if stolen)

What You Need To Do:

  • Step 1: Submit all required documents to the ISPS front desk
  • Step 2: Wait for email from ISPS (if you don’t hear from ISPS after 10 business days of submission, call at 621-4627 to check your application status)
  • Step 3: Pick up your new DS2019 at the ISPS front desk

Note 1: Normal processing time at ISPS is 10 business days after receipt of complete documents
